Any patients attending RIO is taking up a valuable resource designed to help weight loss.  

Do NOT take up a place that someone else could have if you are not fully motivated to lose weight. 

Please download the following and bring them to your first appointment:

    The patient contract 

    The patient wellbeing questionairre (WHO-5) 

    The gym PARQ questionairre 

Appointments are in great demand.

Failure to give more than 24 hours notice of cancellation on repeated occasions may result in discharge from the service.
